Hoc casu documentatur rehabilitatio implantorum per os integrum: duo arcus integri, uterque retentus per vitam, uterque fabricatus ex zirconia ICERA. Imagines captas sunt in laboratorio durante verificatio occlusionis, cum restaurationes collocatae sunt super modella sua operativa et articulatae, ut relatio morsus inspici possit per totum ambitum motus.

Opus in uno arco habet referentiam. Cum arcum superiorem restituere vis contra dentitionem naturalem inferiorem patientis, dentes oppositi tibi indicant ubi occlusio sit ponenda — tu adhibes schemam iam existentem, non novam invenis. In casibus per os integrum haec referentia penitus tollitur. Uterque arcus simul construitur, quod significat dimensionem verticalem, planum occlusionis et schemata directionis omnia esse decisiones quas laboratorium facit, non mensuras quas accipit.


Cur hoc in motu verificatur

Photographia statica non potest ostendere occlusionem. Photogramma unum tantum capitur in una positione, saepe in maxima intercuspatione; et casus videtur rectus in illa positione, sed fallit ubique alibi. Contactus solum fit significans per motum: dum mandibula progreditur in protrusionem, directio anterioris praescripta, disclusio posterioris et schema contactus lateralis per motum aperiuntur.

Verificatio articulata est ubi istae relationes examinantur. Restauratio ponitur super modulos qui sunt montati ut repraesentent relationem maxillarum patientis, et occlusio per movimenta excursiva manu adhibetur. Quaelibet interferentia — ut contactus posterior qui non solvitur durante protrusione, aut contactus non-operativus in parte contraria — hic identificatur et corrigitur, in banco, ubi correctio facilis est.

The alternative is discovering it at delivery. Occlusal correction on sintered zirconia in the mouth means grinding through a glazed surface, and any ground area must be repolished properly or it becomes abrasive against the opposing dentition. Catching interferences before the case ships is not a courtesy to the clinician; it is what protects the restoration.




Material considerations in full-mouth cases

Zirconia opposing zirconia is a specific situation, and it is worth being direct about it. Well-polished zirconia generally shows favourable antagonist wear behaviour, while roughened zirconia can become significantly more abrasive. A roughened zirconia surface is not. When both arches are zirconia, every occlusal contact in the mouth is ceramic against ceramic, and surface quality at those contacts determines wear behaviour for the life of the case.

Primum adiustatio occlusalis perficitur, deinde plena series gradatim politurae; postea glazura adhibetur ubi indicatur pro ultima superficiei forma. Aptitudo passiva per duas arcus

Canalis ad accessum vitarum in imaginibus visibiles sunt, ita positae ut emergant ubi superficies labiales non laedantur. Singulae arcus separatim ad sedem passivam verificatae sunt antequam duo simul articularentur, quia problema aptationis et problema occlusionis in laboratorio similiter apparent — pontifex parum altus propter non plene sedentem sedem tamquam impedimentum occlusionis legitur, et occlusionis correctio ad hoc compensandum errorem perpetuo includit.

Primum aptatio, deinde occlusio verificanda est, ut haec duo varietates separatae manent. Haec ordo tardior est, atque solus est fidus.


Continuitas aesthetica

Casus totius oris exigentiam aestheticam habent quam arcus singuli non habent: duo arcus inter se congruere debent. Tinctura, gradus transluciditatis et characterizatio gingivalis inter superiorem et inferiorem arcum constantes esse debent, quia risus patientis utrumque ostendit. Utrumque arcum eadem copia productiva conficere unam causam variationis tincturae ex materia minuit.


What this case demonstrates

Full-mouth implant rehabilitation sits among the most demanding categories of restorative work.

Two long-span restorations have to seat passively, survive predictable sintering shrinkage, function together through a deliberately designed occlusal scheme, and still match each other in shade and translucency.

The challenge is not any one of those variables in isolation. It is controlling all of them at the same time.

Fit first. Occlusion second. Surface quality throughout.

That sequence matters even more when zirconia is opposing zirconia.
